Workplace Adjustments & Flexibility Review Description: This template supports quick, flexible adjustments that can be reviewed and adapted. Adjustments do not need to be permanent or perfect before being put in place. This template is designed to support early, flexible conversations about workplace adjustments. It can be used to agree immediate or temporary changes that support the colleague in the short term. Line managers can also contact DASS for informal advice at this point. It does not replace formal support planning processes. Where adjustments are ongoing, complex, or require coordination across services, DASS should be involved to support assessment, coordination and formal agreement of adjustments. This document should be used alongside the Life Limiting Conditions Guidance.
